Stormz may correct me, but as I understand it you'll need to finish the Top Gun cert before starting on the Naval Surgeon. As for Ancillaries- good question. Let me get with Stormz on that one.
However, there's a larger topic to look at before you can consider writing for your next cert level: You need to earn the rank of Petty Officer 2nd Class. Until then, setting yourself up through ancillaries is what you'll want to focus on.

Just a note, as I've noticed some people doing this:
Some people have been "pre-awarded" certificates. For every 3 certificates you are pre-awarded, you receive 1 ancillary skill. So, for example, romanflame was awarded 6 certificates, meaning he gets 2 full certifications and 2 ancillary skills total (not two per certification).
If your name isn't on this list, you have to start from scratch. Members returning to active duty will be awarded certificates deemed sufficient by the NCC. Naval High Command

Everyone should also note that Stormz' 1 month deadline for claiming your pre-awarded Certs will pass two days from now. If you have not claimed your certs by then, you will lose the chance to claim them without writing the corresponding missions.
Also, to clear up some confusion for anyone who earns a promotion: excepting cases where free certs are awarded for great contributions to the Navy, any cert you qualify for by promotion you must write out. Only the ones originally awarded when the program opened and any explicitly awarded as free Certs may be claimed without writing the corresponding mission.
As an example: I was promoted from CWO to 2LT, so I now qualify to start my third Cert. Whichever one I choose, I must write an acceptable 4000+ word mission to claim it. (Note: You can split a Cert mission into as many posts as you want, so long as the total reaches the minimum word count required. Include word counts at the bottom of each post.) The minimum word count changes depending on how many Certs you have, with the list being located on the main Naval Certifications wiki page if you're curious.
@Maroy: You should check with Stormz to see whether the Cert you earned when you promoted to CPO is a freebie due to the simulator or whether you need to go ahead and write that last Top Gun mission out.
